Program Details

Cardiovascular disease remains a significant cause of death and disability. There are well identified risk factors for the development and progression of cardiovascular disease and careful treatment and management of these risk factors is a vital key to retarding the progression of the illness.

The diagnostic groups which this program will be targeting are those patients who have an established diagnosis of ischaemic heart disease. This will include those who have a diagnosis of angina or have had an AMI, PCI or CABG’s in the past. The program is not designed to be a substitute for cardiac rehabilitation but to offer a system to provide for the ongoing monitoring of risk factors for those patients with cardiovascular disease.

Key components of the program

  • The CARDIAB data base will be utilised as the tool for data collection, analysis and reporting as it does for the diabetes program.
  • Risk factor goals will be based on the National Heart Foundation Reducing Risk in Ischaemic Heart Disease (RRIHD) 2007 guidelines. These have clear evidence based clinical goals for the management of cardiovascular risk factors.
  • GPs are invited to recruit and register appropriate patients via the Division’s CARDIAB data base in a similar manner to the process for the diabetes program.
  • GPs and Practice Nurses then collect ongoing data and forward this to the Division. This will also be a similar process to the diabetes program.
  • Roll out of the program will occur as a gradual process, firstly targeting practices who have established Practice Nurse positions.

Key aims of the program

  • To increase the utilisation of Medicare Benefits such as Chronic Disease Management (CDM) items, GP Management Plans (GPMP) and Team Care Arrangements (TCA) for this population.
  • To facilitate and educate Practice Nurses in providing ongoing support for people with established ischaemic heart disease in terms of improving their risk factor profile.
  • To provide referral pathways and resources.
  • To provide a resource person in relation to ischaemic heart disease and risk factor modification.
  • To undertake a systematic 6 monthly review and audit of data from the CARDIAB data base and provide feedback on those findings to the GPs.
  • To develop systems for targeting those individuals who have multiple high risk factor profiles and working with GPs to improve these patient profiles.
  • To increase the utilisation of existing services.

The Australian Heart Foundation has just released its updated hypertension guidelines. These can be downloaded from the National Heart Foundation web site  -  Hypertension Guidelines.
