The address for the Southern Highlands Divsion's Diabetes Centre is:

4/20 Bundaroo Street, Bowral NSW 2576.

The entrance is at the front of the buliding and around the left side as you face the building.

Diabetes Education and Management Program:

• Provides diabetes education to people with diabetes referred by GPs. This is undertaken as part of an integrated program with the Health Service Dietetic Program and the endocrinologist.
• Monitors the on-going care of people with diabetes against NH&MRC guidelines.
• Identifies people with diabetes whose data indicates they are at special risk of developing complications. The Program then recommends to GPs that a Care Plan be developed for the patient.
• Encourages people with diabetes to undertake physical activity programs.
• Works with GPs to identify people at risk of developing diabetes.

The Program has been complimented on its achievement of accessing 60% of the estimated number of people with diabetes in the area as well as achieving demonstrated marked improvements in good diabetes control and a marked decrease in poor control.
