CAP- Chronic Care Activity Program
(Previously known as the REO Program)
This Chronic Care Activity Program (CAP) provides medically prescribed physical activity for people with chronic illness referred by GPs. The Division subsidises Healthcare Card holders with a chronic disease into this Program. However, the Division's ability to do this is limited. Existing REO referrals have made the transition into this program.

Example of CAP features
The exercise assessments are being provided by an exercise physiologist at Annesley Lifestyle Centre in Westwood Drive, Bowral.
- 3 months of sessions at the rate of 3 sessions per week
- commencement interview
- commencement tests
- a personalised program formulation in consideration of your present physical condition and referred medical condition
- close supervision and monitoring of each session
- heart rate monitoring of each exercise session
- session log keeping and recording of outcomes
- program variations and changes as required
- interim goal reviews to monitor progress
- communication with your doctor and/or other health professionals as required to ensure benefit from the program
- safety
- exit testing
- case conferencing with your doctor or other health professionals – extra fee applies
- quality information about the exercise that best suits you so you can continue to exercise safely and effectively after your contract finishes
- the opportunity to take part in extra educational courses such as healthy eating, and relaxation.

Referral requires the following to be sent to the Division attention Jill Snow:
1. Referral letter
2. Medication list
3. Medical history
