GP Residential Aged Care Panel Initiative

The Aged Care–GP Panel Initiative was established in 2005 and provides a framework for enhanced communication and collaboration for GPs and Residential Aged Care providers. Development of efficient systems and improving quality of care for residents are key features of the program. Involvement of Allied Health professionals commenced in late 2007 in specific areas i.e. falls awareness and prevention, swallowing difficulties and grief and loss.

Direct referral to HMR pharmacist
