Programs

Nerida provides support to GPs in the area of cardiovascular health and chronic cardiac diseases. She is able to take referrals from GPs for consultations with cardiac patients to assess the patients current conditions, undertake EPC items and provide patient education in relation to cardiac diseases. Following consultations, Nerida provides written reports to GPs and is available to discuss referrals. Please contact Nerida for more information.

New Cardiac Program Coming Soon

The Division is currently setting up a cardiovascular monitoring program that will provide long term assessment of cardiovascular risk factors and facilitate appropriate management. Nerida is undertaking visits to GPs in their practices to provide information about this new program. The program is not designed to be a substitute for cardiac rehabilitation but to provide an emphasis on monitoring risk factor profiles and the development of pathways of management for those patients whose risk factors are not within recommended treatment goals.
